PROGRAM MANAGER (HEALTHCARE FACILITY)

On Lok, a group of five affiliated non-profit organizations is dedicated to providing quality and affordable services for the well-being of the elderly and their families through a full spectrum of healthcare and community programs in San Francisco. On Lok has pioneered the development of the PACE model and is among the most highly respected senior health organizations in the country. PACE (Program of All-Inclusive Care for the Elderly) has proven an efficient and effective model for delivering care to the frail elderly.

POSITION SUMMARY:
Under the direction and supervision of the Director of Associate Director, the Program Manager is responsible for the management of center operations - service delivery, quality of care, staffing and finances.

DUTIES / RESPONSIBILITIES:

  1. Provides leadership in the development and facilitation of participant care planning, care coordination and risk management. Provides QA oversight and insures licensure compliance.

  2. Advances the goals and objectives of the organization by developing supporting department specific performance goals, implementing quality improvement plans with the team, participating in establishing and monitoring financial projections, and working collaboratively with other managers on issues related to quality programming, efficiency and cost effectiveness. Maintains active involvement in the promotion of On Lok in the community.

  3. In collaboration with other managers and specialists, implements center staffing plan, supervises and evaluates staff performance and promotes staff development. Ensures that all IDT members are effectively and actively participating in participant care planning, coordination, problem solving, by providing ongoing coaching and training.

  4. Serves as an effective link with On Lok’s administration, OLSHS management team, and program staff through effective communication, productive collaboration and facilitation of change opportunities.

  5. Other related duties as assigned.

QUALIFICATIONS (knowledge, skills, abilities)

  1. Bachelors degree in Health Administration or Public Health or related. Masters degree preferred.

  2. Two years experience in program operations/administration/supervision/financial management/project management and data analysis required.

  3. Four years experience working with geriatric population.

  4. Experience in tracking, results, timelines, follow up, authority to make changes with the census of group (persuasive)

  5. Skilled facilitator with excellent communication and interpersonal skills.

  6. Ability to bring broad perspective on health and social issues and to bridge these issues on a day-to-day operational basis.

  7. Computer literate, MS office proficient (Word, Excel, PowerPoint)
