Quality Assurance Nurse Associate
Location: San Francisco

PACE (Program of All-Inclusive Care for the Elderly) has proven an efficient and effective model for delivering care to the frail elderly.

POSITION SUMMARY:
Under the direction and supervision of the Quality Assurance Manager, the Quality Assurance Nurse Associate implements, monitors and evaluates the Quality Assurance and Improvement activities of On Lok Senior Health.

DUTIES / RESPONSIBILITIES:

1. Coordinate, participate, monitor, and prepare for various QA Committees including but not limited to:
   a. Quality Management Team
   b. Quality Assurance and Improvement Committee
   c. Infection Control Committee
   d. Dental Review Committee
   e. Pharmacy and Therapeutics Committee
   f. P&P Committee
   g. Home Health Care quality of care review

2. Monitor and evaluate quality improvement opportunities, design and implement individualized falls prevention strategies, and disseminate quality improvement information.

3. Utilization Management and Review including, but not limited to, monitoring and evaluation of the following for quality improvement opportunities:
   a. Participant incident reports
   b. Unusual incident reports

4. Organize and facilitate service utilization reviews.

5. Develop and revise Infection and Exposure Control Plans.

6. Licensure and Audit Coordination.

7. Perform Quality Assurance and Improvement Studies including the collection, analysis and reporting on quality assurance and improvement data and implementation of corrective action plans.

8. All Other Reasonably Related Jobs as Assigned.

QUALIFICATIONS (knowledge, skills, abilities)

1. Graduate of an accredited college.

2. Current and valid California license as LVN or RN

3. At least 3 years experience in health-care preferably in Quality Management, Quality Assurance/Improvement, Utilization management, or Case Management.

4. Experience in Managed Care field preferred

5. Knowledge and experience in Quality Assurance/improvement, data collection, statistical analysis and preparation of reports.

6. Ability to act as an advocate for the frail elderly with all groups served in the program.

7. Flexibility, creativity and ability to work with an ethnically diverse group of clients and employees.
