Licensed Social Worker - Cook Children's Health Plan (Texas)

Summary: To provide social work services to Cook Children's Health Plan (CCHP) members across the health care continuum. Health Plan Social Workers collaborate with Health Plan Case Managers to develop and implement an individualized plan of care that  addresses the medical, psychosocial and educational needs of the member.

Qualifications: MSW/MSSW from an accredited school. Minimum of five (5) years experience (preferably includes some pediatric experience). Bilingual in Spanish desirable. Must be able to: 1) communicate effectively orally and in writing; 2) organize and prioritize multiple activities; and, 3) work with individuals from diverse cultural, social and economic backgrounds. LMSW licensure required.
