**Managed care penetration rate calculated as a percentage of state population per U.S. Census Bureau estimates. Managed care penetration is calculated by the number of enrollees reported at the state level for health plans that are able to provide a geographic breakdown. State-by-state breakdown of enrollment was not available from all plans. Partial state breakdowns are used where only part of the total plan enrollment can be attributed to a specific state. Enrollment is often counted by the location of the employer/group health client or plan sponsor, even though the plan sponsor may have employees/members located in other states (as in Washington DC). This can result in a managed care penetration of greater than 100%.
